CDMS (Chronic Diseases Management System) is a clinical specialist system used by healthcare professionals in day-to-day patient care and has modules for, e.g., diabetes, cardiovascular and heart diseases, COPD and other chronic respiratory conditions. The system integrates with and can be used as an extension to a hospital EMR.
CDMS has been designed to support the work of multidisciplinary care teams and integrated care between hospitals and community. The system gives a holistic view of patients’ current medical condition and history, and enables early identification, assessment and intervention to prevent complications from developing, hence reducing the need for hospitalization.
